Attention all Nurses and Nursing Students!
It is so critical that we keep our patients stories organized, manage our time efficiently, and communicate clearly so that we directly enhance patient recovery. Even the most seasoned nurses use a report sheet of some type to make sure they are passing along all critical information to oncoming shift. I have updated this post and included a new PDF of the exact report sheet I use every single shift and patient. There is a lot going on and ratios are not always ideal. Manage your time wisely and communicate effectively.
Why is report important?
I believe that aside from excellent assessment skills (and after critical thinking) the next best skill for a nurse to have is being able to give effective report. This means you are able to give a short but comprehensive verbal picture of your patient to another nurse. Change of shift is a critical time. You want to leave at the end of your shift and the oncoming nurse doesn’t want to start late.
How to Master “Hand-off”
A quick but full hand off report is something to practice and master. Few hospitals use paper SBARs (or report sheets) that nurses update and hand to each other for report. Some hospitals solely rely on the verbal report between nurses. To help the new grads, nursing students, and nurses who may be struggling with hand off, I made an easy to use print out. I have created this helpful resource that you can fill out for each of your patients.
If you use it often, it will help you give effective report by practicing how to verbally describe your patients story.
